Default Gas Conversion Rebate Form

hey just wanting to know where you get this form from as my car is booked in for gas this wednesday and i would like to get my rebate back as soon as possible. thanks!

The people doing the conversion have to sign it so they should be filling it in and handing it to you, I would have thought, I think you can then submit it at any medicare office (well they have posters up advertising the rebates)
